BE 碱剩余(base excess)是指在标准条件下滴定血标本,使其pH达到7.4
需要加酸的碱剩余为正值
需要加碱的碱剩余为负值 (代谢性酸中毒)
AG 阴离子间隙 (anion gap) 是指血中未测定阴离子与未测定阳离子的差值
代谢性酸中毒的原因有两种
一是体内固定酸产生过多,碳酸氢根离子因为中和而减少 这种的阴离子间隙增高血氯正常
比如说乳酸酸中毒、酮症酸中毒、肾衰竭(固定酸排泄障碍)、阿司匹林
二是碳酸氢根离子直接丢失
这种的阴离子间隙正常血氯增高
胃肠道比如说严重腹泻、12指肠引流、肠瘘等,由于肠液,胰液和胆汁中碳酸氢盐含量高于血浆,大量碳酸氢盐从肠道丢失
肾脏碳酸氢根离子在近端肾小管重吸收减少,随尿排出增多
碳酸酐酶抑制剂如乙酰唑胺,抑制碳酸酐酶活性,肾小管泌氢离子和重吸收碳酸氢根离子减少
高钾血症时,钾钠交换增强,氢钠交换减弱。(肾小管每排出一摩尔氢离子,就会有一摩尔碳酸氢根离子重吸收。 )肾小管泌氢离子减少对碳酸氢根离子的重吸收减少,导致代谢性酸中毒。由于肾泌氢离子减少,尿液呈碱性,故称为反常性碱性尿钾离子与细胞内氢离子交换引起细胞外氢离子增多
